you'll have to change the
root (/dev/disk/by-id/scsi-SATA_ST9402112A_5PJ1X5AL-part5)
to
root (hdX,Y) where:
X is the number of partition, starting from 0 and Y is the partition, also starting from 0
this is a bug with one kernel wich I don't remember now....
