Thanks for report! Your filesystem has obviously suffered a corruption. Did
your system experience power failure recently?
I suppose you did not record the filesystem image before you ran fsck? And I
also suppose you don't see errors anymore after fsck?
If this happens next time, please run: e2image -r /dev/md-0 - | bzip2
/usbdisk/ext4-dm-0.bz2
before running e2fsck. It will dump metadata from the corrupted filesystem for
future analysis. Debugging filesystem corruption without reproducible test case
is hard but debugging it without fs metadata is practically impossible...
