I'll try to install KDE and reproduce.

Just did with fresh KDE installation and freshly created account and it works fine for me - pinentry-qt4 window displays on top with focus. Jim, could you please post output of 'rpm -qa | egrep 'pinentry|kmail|libqt4' here?
