I don't see any add call. There is only remove call:
/sbin/update-bootloader --image /boot/vmlinuz-2.6.34-12-default --initrd
/boot/initrd-2.6.34-12-default --remove --force
interesting line is this one (few times in log, maybe bad name?):
update-bootloader: no /boot/vmlinuz-2.6.25.20-0.5-default found
There is no section with image '/boot/vmlinuz-2.6.25.20-0.5-default'
Usage:
update-bootloader [operation] [options]
operation is one of --add, --remove or --refresh.
valid options are --help, --man, --image <file>, --initrd <file>,
--xen-kernel <file>, --xen, --default, --previous, --name <string>,
--force, --force-default.
