I think something is loading the module directly by name. Can you confirm that
/sbin/modprobe pci:v0000168Cd00000024svXXsdXXbcXXscXXiXX
does NOT load the module, while
/sbin/modprobe ath5k
does? Then you could try replacing /sbin/modprobe with something like
#!/bin/bash
case "$*" in
*ath*)
echo "$0 $*" >/dev/tty1
ps -fw $PPID >/dev/tty1
esac
exec -a modprobe /sbin/modprobe.bin "$@"
and try to find out what script is loading the module.
