Unless something else is going on here, the device-mapper rules need to be
fixed.
In any case, even if this is not the problem here, device nodes must never be
deleted by any udev rule. It is a very broken idea of device-mapper upstream
how hotplug/udev/the driver-core interacts.
We also, for very good reasons, don't do any "STARTUP" flagging in udev rules.
This can never work reliably on today's systems.
