Probably it is not even a rmmod / insmod that's required, it might be that a

hciconfig hci0 reset

(as root) is enough to get it going again, so coolo, please try this first.

If this helps, then i can put a pm-utils hook into the bluez-utils package (it
belongs into the kernel package, to be exact, since it is a broken kernel
driver, but i will take it into bluez anyway :-)
