I rebooted to make sure the new mount options are propagated, and deleted the
quota files. The system is not robust:
# /etc/init.d/boot.quota restart

It seems quotacheck can't read the format from the mount options. Unfortunately
man quotacheck doesn't say what the correct name for the format mount option
would be (another bug).
Oops,
# quotacheck -F vfsv0 -m /
fails to produce quota files after finishing the disk scan without error.
This whole quota scheme doesn't currently seem to work at all :((
