It sounds like you might be confusing updates (patches) with upgrades (new
versions of packages available in repositories). The updater by default will
only display updates. You can right click -> configure -> show available
upgrades if you want to see those too.

Traditionally only updates are notified. In 10.1 and 10.2(with zmd) the
upgrades were shown as well which was really a bug.

zypper will behave the same, "zypper up" will only apply patches. "zypper up -t
package" will do upgrades too. Upgrading all packages is rarely a good idea,
more likely you might want to restrict it to a specific mirror with "zypper up
-t package -r KDE4" for example.
