I finally tracked down the problem to my installing/removing PulseAudio updates (specifically pulseaudio-0.9.8-11.1) which create the file /usr/share/PolicyKit/policy/PulseAudio.policy which polkit-policy-file-validate considers invalid. When this file is present and an SD card is inserted, for example, the debug output of hald gives: ** (process:6680): WARNING **: Error loading policy files from /usr/share/PolicyKit/policy: /usr/share/PolicyKit/policy/PulseAudio.policy:29: parse error: parsing aborted .. 23:23:44.667 [I] hald_dbus.c:4025: failed with 'org.freedesktop.Hal.Device.PermissionDeniedByPolicy' 'hal-storage-mount-removable no <-- (action, result)' When this file is removed, everything works flawlessly. The bug being filed is that a malformed .policy file should not cause hal operations to fail -- at least not without a much more informative error message.
