Well I think I've narrowed it down.  It turns out that it works as long as the
Nvidia driver is not loaded, ie it works with plain old 'nv'.  It was working
with the non-default kernels as I wasn't using the binary nvidia driver with
those, I might try getting the shell installer and kernel source and installing
it that way instead.
In short I doubt it's a kernel issue, seems that the disk etc is woken-up but
Nvidia (from http://download.nvidia.com/opensuse/10.3) still wants more sleep
:)
