Your crypttab entry is wrong (s/luks/swap/). I doubt that you actually used
YaST to set up the encrypted swap partitions as YaST does the correct thing:
# cat /etc/crypttab
cr_sda12 /dev/disk/by-id/scsi-SATA_IC35L080AVVA07-_VNC402A4D3DEAA-part12
none swap
# grep cr_ /etc/fstab
/dev/mapper/cr_sda12 swap swap defaults 0 0
# /lib/udev/vol_id
/dev/disk/by-id/scsi-SATA_IC35L080AVVA07-_VNC402A4D3DEAA-part12 | grep TYPE
ID_FS_TYPE=crypto_LUKS
# /etc/init.d/boot.crypto start
Activating crypto devices using /etc/crypttab ...
Please enter passphrase for
/dev/disk/by-id/scsi-SATA_IC35L080AVVA07-_VNC402A4D3DEAA-part12 (cr_sda12):
key slot 0 unlocked.
Command successful.
Setting up swapspace version 1, size = 4005146 kB
no label, UUID=dee2a1b3-8602-41c3-9161-b1dbee713125
/dev/disk/by-id/scsi-SATA_IC35L080AVVA07-_VNC402A4D3DEAA-part12... done
Order of init scripts also is correct. boot.swap requires boot.crypto.
