There is no instance of /usr/X11 in this document.
It used to be the common directory for X11 installations before R6 came along. X11R6 was introduced to keep two releases on a system in parallel.
in the Linux/OSS world, /usr/X11R6/... looks being used mostly (IMHO unfortuneately), most other systems' common path is /usr/{bin,lib,XYZ}/X11. since the latter is mentioned in FHS, my only remaining concern is about /usr/X11R6/whatever -- mainly /usr/X11R6/bin
OPENSolaris uses /usr/X11. /usr/X11R6/ is a link to it, /usr/bin/X11 links to /usr/X11/bin. Open/Free/NetBSD all only use /usr/X11R6. No /usr/X11 and no /usr/bin/X11.
