Nope it has never worked. Using microsoft Intellimouse ps/2 mouse with a wheel. I have only configured using Sax2 using the Nvida modules. Make a save copy of /etc/XF86Config, then try replacing your "pointer" section with this: Section "Pointer" Protocol "IMPS/2" Device "/dev/psaux" SampleRate 60 ZAxisMapping 4 5 Buttons 3 BaudRate 1200 EndSection then restart your X server by either logging out or toggling init levels. I have a PS/2 Logitech 2 button wheel mouse and this works for me as well as enabling the wheel for scrolling & as button-2 when it's pushed.
