Nothing wrong with that setup. It is true that any IDE device should work on any IDE port, but operating systems get confused by egregiously non-standard setups like the OP apparently has: no master on 0, a cdrw as slave on 0 and a harddisk as master on 1.
I don't see anything wrong with your setup from the cabling / jumpering point of view. DVD readers are cheap enough now that I'd think of getting another to swap in there to test by substitution.
The received wisdom years ago was that it was better to put hard disks together on one cable and CD type devices on another, but I don't think that's much of an issue anymore since CD devices are all ATA-33 now.
Try hdparm -iI /dev/hda then /dev.hdb etc. and see what you see.
