It is complaining about /dev/sdd, which does not exist. There was one sdd, external (esata), about 18 hours ago. Disk had an error, was removed and connected on another cable. Now that I re-read comment #1, my analysis is wrong. Those traces aren't tainted D - it's that fdisk is in D state. Going through too many bugs, I'm afraid. The log does help, though, as it documents the condition of the failed disk. So it probably is the case that we are waiting for IO (it must be read) that is never completed as Jeff suggested. Carlos, given the nature of the problem, I suppose you are not able to reproduce the issue, right? In case it happens in future, could you try doing: echo 1 >/sys/block/sdd/device/delete and see whether it makes things going? In case it happens in future, could you try doing: echo 1 >/sys/block/sdd/device/delete and see whether it makes things going? It tells the kernel that the drive is actually gone...
