I'd appreciate any debugging from you two :), because I can't reproduce the bug
myself and am out of ideas as to why it happens.
Does your initrd contain the fixed modprobe.conf?
$ zcat /boot/initrd | cpio -i --to-stdout etc/modprobe.conf | grep hcd
Does it contain ehci-hcd.ko?
$ zcat /boot/initrd | cpio -t | grep hcd
Can you try changing the install rules to
install uhci-hcd set -x; exec &>/dev/console; /sbin/modprobe -v ehci-hcd; \
/sbin/modprobe -v --ignore-install uhci-hcd
install ohci-hcd set -x; exec &>/dev/console; /sbin/modprobe -v ehci-hcd; \
/sbin/modprobe -v --ignore-install ohci-hcd
Hmm, looking at it now, it *could* be that two modprobe invocations race, one
starts inserting ehci-hcd, another thinks that ehci-hcd is already loaded even
if it's not (this should not happen, but...) and loads uhci-hcd right away. Or
modprobe ehci-hcd returns an error and ohci-hcd is loaded first.
If you have the possibility to boot with a serial cable attached, please try
the above, it could help us understanding what's going on.
Re comment #28 - this is because mkinitrd runs depmod in the initrd, it doesn't
copy modules.dep from the live system.
