There is an entry 'CDMA Connection' found on the NetworkManager-gnome menu. Selecting this entry causes the NM applet to spin maybe a minute or so (until timeout?). But no dialup connection is established, no connection lamp that illuminates, just the waiting lamp twingkling on the modem. I don't know other ways to 'Stop NetworkManager'(?), so I selected instead traditional network setup with ifub in YaST to stop NM.
Below are attached the two terminal logs.
The 'Stop NetworkManager' means just kill the NetworkManager, and run with "NM_PPP_DEBUG=1 NetworkManager --no-daemon" in terminal. Before it use kill the modem-manager and run again with '--debug' in terminal. Then try to connect the CDMA's connection. Please attach your log again. From your hwinfo I find the Product id still is 0x6803, it should be 0x680a which is changed by usb_modeswitch.
So this device probably requires the fake-cd eject procedure done with usb_modeswitch. Could you eject your fake-cd? Wait a little itme, is that working? Once that's done, you'll have the tty ports, and modem-manager should at least start talking to the device.
