Hmmm... Now you have timeout on FLUSH_EXT and then IRQ delivery seems gone.
Timing out on 0xEA is usually a pretty good indication that there's a power
related problem although in this case it doesn't quite match the usual pattern
in that IRQ delivery seems to be lost afterwards. Can you please try the
following two things?
1. Boot with "irqpoll". The problem will happen again but the system might be
able to recover and just keep operating. If that happens, please attach the
output of dmesg.
2. Try a different power supply. I usually find connecting only hard drives to
a separate power supply very helpful in isolating power related issues. You
can power up a second power supply without motherboard by following
instructions on the following page.

It's perfectly safe to use separate power supplies for SATA devices but please
don't connect PATA device to a separate PSU. If this seems like a bit too
much, just replacing the power supply with a different one and seeing what
happens should tell us a lot too.
