There is no entry in /etc/fstab, so it's probably automounted. Removing and reinserting achieves nothing.
Not sure what you are talking about. Of course there is no fstab entry. Hal mounts it when inserted. Chmoding those dirs cannot work when the device is readonly and anyway should be done via the gui you are using that knows how to change settings via hal.

I did an CHKDSK /f E: on it returning:
The chkdsk output is fully pointless as it says exactly nothing about the partitiontable but only of the filesystem on top of it. So it still is possible your partitiontable has problems. And i still miss a clear answer to when exactly (sd card in what state.. readonly or not) which problem occurs. Please also post again the dmesg output of your problem-situation _without_ using grep as it hides the context of the problem. So it now behaves rather normal except for the
mmcblk4: unknown partition table
so, then from what state is the log from comment #6 where it worked without any problems?

The interesting things would be your "lspci -vnn" output as info about your machine. Hm? But in comment #12 you just posted that with write protect being OFF it would also report "mmcblk4: unknown partition table". So, is this only a error seen from time to time are always present?
Alas I don't have any other card reader available, this one is built in. I am using three MMC cards of the same size and brand, they all produce the same result.
Ok, then please also provide the mbr's of the other two cards also.
I am attaching the mbr extracted with
dd if=/dev/mmcblk0 of=mbr.bin bs=512 count=1
for your perusal.
