Firewall blocking incoming ports. Turn on debug logging in postfix. Posting
the relevant part of postfix.conf so an expert can look at it would also be
helpful.

I had postfix pipeing through my ISP's smtp server (verizon). Well, I
just tried to telnet verizon's smtp server on port 25 to test it, and
got connection refused. I'm pretty sure they finally changed their
setups.
